The Patient Portal at San Tan Cardiovascular (STCV) Center is a secure method for patients to access, communicate and receive information related to their electronic health record (EHR) online.

Patients who are “web-enabled” may access the portal securely with a username and password.

The Patient Portal is an easy way to get in touch with your healthcare provider. It is a secure, internet application that allows you to:

  • Submit requests for medication refills.
  • Access your visit summaries from each encounter at STCV.
  • Access your personal health information contained within the EHR including information about labs, diagnoses, medications, immunizations, family medical histories and treating provider.
  • Request information, such as an appointment with a provider at STCV, and reschedule or cancel an existing appointment.

How can I get a Patient Portal account?

All you need is a current email address! At your next appointment, ask a staff member to enroll you in the Patient Portal.

Why should I use the Patient Portal?

Sometimes, it might not be convenient to call our office. Other times, you may have a non-urgent question and think an appointment isn’t needed. With the Patient Portal, you can review visit summaries, results and appointment information on-line at your convenience.

How do I get to the Patient Portal?

A STCV staff member will enroll you and activate your account as being “web-enabled” in our system. They will provide you with instructions for how to log in to the Patient Portal via the Internet. It is as simple as clicking the “login” button and typing the username and password that is provided.

How do I know my health information is safe?

STCV has made significant investments to secure your information. We currently meet all regulatory requirements for security of our network and data. Information on the Patient Portal is protected as part of your electronic health record.

Can I use the Patient Portal if I have an urgent need?

The portal is to be used for obtaining general information about your healthcare. It is NOT to be used for urgent questions or emergency situations that require contacting your healthcare provider.

Can I print from the STCV Patient Portal?

You are able to print information from the Portal. Anything you print from the Portal is your responsibility. To protect your privacy, keep these papers safe.

What do I do if I forget my username or password?

In the event you forget your password, click on the “Trouble logging in” link that is on the login section of the home page. You will be routed to the Account Recovery Help page and prompted to select an option to recover your portal account: “Forgot Username” or “Forgot Password”.

If you select “Forgot Username”, you will be prompted to enter your e-mail address and answer the security question entered when you created your account. enter your e-mail address and answer the security question entered when you created your account.

If you select “Forgot Password”, you will be prompted to enter your email address and answer the security question that you entered when your account was created. For security purposes, you will be emailed a verification link and asked to confirm your new password again in order to complete the password reset.

It is your responsibility to keep your username and password safe. STCV is not responsible if you inadvertently misplace or lose your username or password and someone else uses the password and views your private information. STCV will never ask for your password. If someone calls and asks you for it, do not give them your password.

What do I do if I still have trouble logging in?

In the event that you have trouble logging in, please contact our office to speak with one of our staff.
